This could be a doctor’s note, a digital laboratory requisition, or a physician’s office printout. The note must have this information, the tests that will be drawn, the diagnoses codes, physician’s name, address, phone, fax, physician’s NPI number, and the requested laboratory account information for processing (if applicable) **Laboratory WILL NOT process tests or release results unless there is a physician associated with the order.**

Also referred to as a “face sheet”, this has the patient full name, date of birth, service address, contact information, or caregiver / service coordinator contact information. May also include insurance information. **Patient or party will be called directly to confirm availability, please be sure contact information is current.**
